On an HPE system configured with Broadcom NetXtreme adapters, when the system is set up for iSCSI Boot from SAN (BFS), the Windows Server 2019, 2022 or 2025 installation will complete successfully but the system will encounter a Blue Screen of Death (BSOD)(Stop code: INACCESSABLE BOOT DEVICE) during the first reboot.The operating system will not call the AddDevice rule for the interface, and, as a result, the driver will not be loaded, causing connection failure between the iSCSI initiator and the iSCSI Target. The BSOD observed with INACCESSIBLE_BOOT_DEVICE is a known issue and is an expected behavior for device ID change case.

This is a known Microsoft Windows issue with iSCSI configuration. The operating system will not accept a Device ID change for the iSCSI Boot Firmware (iBF) Table (iBFT) interface post-OS installation.To avoid this issue, load the Out-of-Box (OOB) driver 219.0.1.0 (or later) during the OS installation. The driver is available as a component ofSPP version 2021.10.0.For Windows Server 2025, the minimum supported OOB driver is 221.0.6.0. After the OS installation is completed and booting into the OS is successful, do NOT rewrite the OOB driver, the system will still encounter a BSOD (Stop code: INACCESSABLE BOOT DEVICE).If the BSOD occurs, use F8 > "Advanced boot menu" and select "last known good configuration" and the system can boot to WS2025. This should be a short-term work around.
