In some instances, HPE Fibre Channel QMH2672 Host Bus Adapters running in an HPE Superdome X environment may prevent a system from booting into an OS. In order for this issue to manifest itself, the adapter MUST be running firmware version 8.08.230 or later AND be connected to only one or to no interconnects. If these conditions are met, then during every boot, the adapter will cause a UEFI message to appear, noting that the system is being reset to apply configuration changes before booting the OS. If the system is running in Legacy BIOS Boot mode, it will still be affected by this issue. The issue is specific to a QMH2672 UEFI driver with Superdome X servers and empty interconnect bays.
This issue affects QMH2672 adapters running firmware version 8.08.230 or later. This firmware is included in the following versions of the QLogic firmware update smart components: 2019.12.03 2019.12.05 2020.09.01 Additionally, the adapter must be installed into a Superdome X server platform and the adapter MUST be connected to at least one empty interconnect bay in the enclosure. To view the current firmware version of an adapter in a Superdome-X system experiencing this issue, perform the following: 1. Boot to System Utilities from HPE Integrity Superdome X Onboard Administrator OR From a UEFI shell, enter an "exit" command to launch a Boot Menu or System Utilities(menu interface). 2. From the Boot Menu, enter U/u to Launch System Utilities(menu interface). 3. From System Utilities, select Device Manager. 4. From the Device Manager Devices List, select HPE QMH2672 16Gb 2P FC HBA - FC. 5. From HPE QMH2672 16Gb 2P FC HBA- FC, select Adapter Information. 6. View the Adapter Information to find the firmware version details.
This issue will be addressed in a future release of QLogic firmware. As a workaround, if this adapter is installed in a system where it is not connected to ANY interconnects, remove the adapter from the system. Systems in which the QMH2672 is attached to a single interconnect should remain at firmware 8.08.203 (in version 2019.03.05) or earlier to avoid this issue until a new firmware is available.
