An ESXi server being upgraded to ESXi 6.7 may experience a Purple Screen of Death (PSOD) if HPE StoreVirtual Multipathing Extension Module (MEM) version 12.6 is installed and active at the time of the upgrade.

Scope
This issue affects the following HPE StoreVirtual product families when using HPE StoreVirtual MEM for VMware in a VMware ESXi environment:
HPE StoreVirtual 4000 Storage
HPE StoreVirtual VSA Software
Resolution
HPE requires that you uninstall the StoreVirtual MEM and reboot the server prior to upgrading the ESXi server.
If the server was updated to ESXi 6.7 prior to the removal of the MEM, you must uninstall the MEM and reboot the server. Then install MEM version 6.7, and reboot the server again. If the system experiences a PSOD, reboot the server again until the error clears.
Complete the following steps prior to upgrading the ESXi server:
Change the Path Selection Policy from HPE_PSP_LH to something else (example: Round Robin).
Uninstall the StoreVirtual MEM and reboot. It may be possible to receive a PSOD during reboot, if so, reboot again until the error clears.
Upgrade the ESXi Server.
Install MEM version 6.7 and reboot.
Change the Path Selection Policy to HPE_PSP_LH.
