When SUSE Linux Enterprise Server (SLES) 16 is running on an HPE Synergy 480 Gen11/Gen12 Compute Module that boots from a Fibre Channel Boot LUN via any of the HPE Host Bus Adapters (HBAs)/HPE Converged Network Adapters (CNAs) listed in the Scope section below, performing an eFuse or power cycle reset may cause the operating system to report EXT4 filesystem errors and enter emergency mode.Typical error messages observed:EXT4-fs error (device dm-5): bad orphan inodeEXT4-fs error (device dm-5): Orphan file not empty on read-only fs.EXT4-fs (dm-5): mount failed

As a workaround, perform any of the following preventive procedures:PROCEDURE 1: Add a kernel parameter to force filesystem check on bootAdd the following parameter to the kernel command line:fsck.mode=forceNOTE: This option forces a filesystem scan on every reboot, which may significantly increase boot time on systems with large file systems.ORPROCEDURE 2: Convert the filesystem to btrfsConvert the existing EXT4 filesystem tobtrfs. Instructions are available atConvert - brtfs-convert tool instructionsNOTE: Back up all important data before proceeding with the conversion.ORPROCEDURE 3: Reinstall SLES 16 with the default btrfs filesystemPerform a fresh installation of SLES 16, selecting the defaultbtrfsfilesystem option during setup.RECOVERY OPTIONSIf the Operating System has already entered emergency mode, perform either of the following procedures to recover the system:PROCEDURE 1: Manually clear the orphan fileLog in as root when prompted.Execute the following command, replacing X with the appropriate device number (for example, dm-5):e2fsck -fp /dev/dm-XEnter exit to continue booting.ORPROCEDURE 2: Perform a cold bootPower off the system completely and power it back on.
