Symptom
Following alerts might be observed on a Cisco VG400 voice gateway randomly:

Apr 9 20:03:40.182 gmt: %ENVIRONMENTAL-1-ALERT: RPM: fan0, Location: P2, State: Warning, Reading: 3420 RPM
Apr 12 13:48:40.559 gmt: %ENVIRONMENTAL-1-ALERT: RPM: fan1, Location: P2, State: Warning, Reading: 2940 RPM
Apr 22 14:00:08.361 gmt: %ENVIRONMENTAL-1-ALERT: RPM: fan1, Location: P2, State: Warning, Reading: 3060 RPM

Workaround
The problem goes away by itself usually. It's possible a warmer ambient temperature will eliminate the alarm as well.
Further Problem Description
The alerts are not specific to any particular device and the exact trigger is not well understood. However the low speed alarm threshold is set to an incorrectly high value and that will be corrected by this bug fix.
