Conditions
Switch reports 'Stack Adapter Authentication Failed' on bootup.
No trigger.
Issue occurs on standalone switch as well if stack adapter is connected.
Show inventory output will list stack adapter as part of switch.

Sometimes, the issue occurs when the switch is added to stack and then disconnected.
Workaround
Power cycle of the switch helps to recover the switch from this scenario.
