Symptom
A catalyst 9410R running 16.12.4 (install mode) and being upgraded to 16.12.5 via ISSU may crash if "debug issu all" is ran. The standby processor will upgrade and then the active processor will crash. The upgrade will ultimately complete.
Workaround
Do not run "debug issu all"
Further Problem Description
Has been also seen on 16.12.1c with SMU to 16.12.5 upgrade. Could also be seen on other codes. The key appears to be the "debug issu all". A warning message is provided when the debug is engaged that it is an intensive command.
